Fundraising Spotlight - Black Swan Health - Freo Street Doctor
One of the groups given assistance by our 2020 charity partner, Fremantle Foundation, is Freo Street Doctor. It is a mobile General Practice providing primary healthcare and mental health support to those people most at need and vulnerable in our community. Primary health care services are provided by General Practitioners, supported by Registered Nurses and Outreach Worker/Counsellors. The team ensures physical health and medical care is provided, as well as counselling and linking patients with complementary social services needed to support the complex social needs of clients.
For 94% of its clients, Freo Street Doctor is their principal source of health care. 64% of Freo Street Doctor clients have two or more co-existing chronic health conditions, 22% have three or more chronic co-existing conditions and 85% of clients have a complex mental illness. Without Freo Street Doctor many of these people will be unable to access physical and mental health care.
Freo Street Doctor clients include the homeless or those at risk of homelessness; Aboriginal and Torres Strait Islander people; people experiencing financial disadvantage; youth; people with diagnosed and undiagnosed mental health illness.
